My Aged Care

Changes to aged care services:

From 1st July 2015 people over the age of 65yrs who wish to access aged care services will be required to go through My Age Care.

This is a centre that will assess and register your care needs, the centre will build a profile on your aged care requirements and refer you either to a Regional Assistance Scheme (RAS) assessor or other services such as ACAT team.

The RAS will then connect people with relevant services such as transport, meals, home modification, or other home care services.

Example – if you need transport post 1 July 2015 you will need to register with My Aged Care and then you will be referred to a transport provider in your area i.e. Community Transport.

Community Transport will then register you and provide the service

Registration with My Aged Care is a once only exercise.
