To register for our service:

If you are over 65 you will need to contact My Aged Care http://www.myagedcare.gov.au/ or call 1800 200 422

If you are under 65 you are able to register directly with our service by either calling one of our offices in your local area.

We will then send you out a Self Referral Form, this form, will need to be filled out and returned or just click on this link for the form and print it out and return. On receiving your form a staff member will assess your eligibility and how we can help you.

You must be registered prior to requesting transport, so you don’t miss out, as we need a minimum of 2 working days’ notice to organise your booking. The more notice you give the better.
